To qualify for a settlement payment, you must complete a Claim Form either submitted online or postmarked on or before March 17, 2025.

To start your online Claim Form, you can find your Notice ID & PIN printed on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator at (866) 675-2852. Please request your Notice ID & PIN by providing the Class Member’s full name and mailing address.


Claim Forms may be also be downloaded here, and mailed to the Settlement Administrator at the following address: Minor v. Oldcastle APG, Inc., P.O. Box 25226, Santa Ana, CA 92799.
